FAMILY COMMUNITY SUPPORT SYSTEMS
1. Plan
for you visit by preparing questions to ask about:
a. the family’s perceptions of the community’s strengths and areas needing
improvement.
b. family involvement in the community.
c. community services the family uses (e.g. church, school, recreational
facilities).
d. use of health care services and types of providers each family member
sees (fee, low cost, private, or group).
e. satisfaction with health care services and providers.
f. experience with, knowledge of, and plans for use of emergency services
(e.g. medical, fire, police etc.).
g. family’s experiences with, attitudes about, and expectations of a
professional nurse.

During the visit:
a. Use your prepared questions as a guide for discussion.
b. Ask if there is anything else the person wants to discuss.
c. Summarize the conversation and end the contact in a professional
manner.
3. Documentation analysis of your visit is to describe and explore:
a. The family’s perceptions of the community’s strengths and areas needing improvement.
b. Family involvement in the community.
c. Community services the family uses (e.g. church, school, recreational
facilities).
d. Use of health care services and types of providers each family member
sees (fee, low cost, private, or group).
e. Satisfaction with health care services and providers.
f. Experience with, knowledge of, and plans for use of emergency services
(e.g. medical, fire, police etc.).
g. Strengths and weaknesses of the family in the area of community
support.
h. Family’s experiences with, attitudes about, and expectations of a
professional nurse.
i. Your feelings and reactions during the visit. (Reflections)
j. How you perceive these visits to a family help you develop your
understanding of the role(s) of the professional nurse.
k. What you did well and how you would improve your family visits.
