Family – Health Definition

 

 

 

A.         Questions to guide your visit:

                                                                                                                         

1.               How do you define health?  How does your definition of health and health  
          beliefs affect the rest of the family?

 

2.                  How does your family perceive their health?

 

3.                  What kinds of health practices do you practice?  Your family?

 

4.                  What kinds of health practices would you like to practice but are not now practicing?  What is stopping you from practicing these?

 

5.                  What do you do when someone is ill?  How does that affect the rest of the family?

 

6.                  Is there any health promotion/disease prevention practice you would like to learn more about?

 

B.                 Written Record of the First Visit

 

1.                  In narrative form discuss the following:

 

a.       The primary contact’s definition of health and health beliefs and how these beliefs affect the rest of the family.

 

b.      The family’s perception of their health.

 

c.       Each family member’s health related activities.  Discuss how each family member’s health related activities are either health promoting behaviors or disease prevention behaviors.

 

d.      The health promotion/disease prevention practice that the primary contact would like to learn more about and why this is important to the individual.

 

e.       Strengths and weaknesses of the family in the area of health beliefs and activities.

 

f.        Your feelings and reactions during the visit.  (reflection)

 

g.       What you did well and how you will improve the next visit.  (self-evaluation)

 

2.         Submit the report by the scheduled due date.
